CALENDAR OF ATHLETIC EVENTS

$30.00 Sports Fee must be paid prior to the

1st game/match/meet to be eligible to compete.

SPORTS FEES FOR 23-24 SY CAN BE PAID AS OF AUGUST 1, 2023

You can pay the school bookkeeper or pay online through Infinite Campus

6th grade activities available

Academic Team, Cross Country, Dance Team, Tennis, Track & Wrestling

7th and 8th grade activities available

Academic Team, Basketball - Girls & Boys, Cross Country, Dance Team, Football, Tennis, Track, Volleyball & Wrestling

**The middle schools have instituted a cut policy for Volleyball, Basketball, Dance, Academic Team & Tennis.

Athletes must have had one(1) physical during their time at Summitvue. If your athlete has NOT HAD a physical, you still need to print out the Physical Exam Form and have that completed by a medical professional. Physicals will be uploaded onto the website.

Requirements to Participate in Extracurricular Activities

Activity & Athletic Eligibility Checklist must be completely filled out (online using the forms above) and signed by both the athlete and parents/Guardians.
A Physical Exam Form must be current and on file with SMS before the first practice OR brought to the first practice.
Consent /Interim Questionnaire/Emergency Contact form needs to be filled out each year.

If an athlete is not riding the bus after an event, and will be riding with someone other than the parent/guardian, this Transportation Form must be completed.
